Horizon Pharmacy & Medical Clinic: Bridging healthcare gaps in Mill Woods
Located within the innovative Lakewood Covenant Community Health Centre, Horizon Pharmacy & Medical Clinic represents a powerful combination of pharmaceutical expertise and primary care, serving the diverse Mill Woods community with integrated healthcare solutions. This unique partnership brings together over a decade of community pharmacy experience with an established family medicine practice under one roof.
A decade of community-centred care
Janak Patel, co-founder and pharmacist at Horizon Pharmacy, brings extensive experience and deep community connections to this groundbreaking healthcare setting. With over 12 years as a practicing pharmacist, Janak has built a reputation for advancing patient care across southeast Edmonton through his founding of Tamarack Community Pharmacy, Summerside Value Drug Mart and CareMed Wellness Inc.
"I've been working as a pharmacist for a good 12 years now," says Janak. "Through my work at various locations across southeast Edmonton, I've been fortunate to build relationships with many family physicians in the area and focus on ways we can better serve our patients together."
Having this extensive network of professional relationships positions Horizon uniquely to provide seamless, collaborative care within the Lakewood Covenant Community Health Centre's integrated model.

Established practice, new location
What sets Horizon Pharmacy & Medical Clinic apart is its foundation of established patient relationships, proven care delivery and seamless integration between pharmacy and medical services.
“We have relocated the long-standing All-Well Primary Care Centre to the new Lakewood Covenant Community Health Centre and renamed it as Horizon Medical Clinic,” says Janak. “The clinic is now owned and operated by Mrs. Ankur Arnold, continuing the same trusted team of physicians and staff that have served the Mill Woods community for over 20 years.”
Alongside the medical clinic, Horizon Pharmacy plays a central role in supporting collaborative patient care. By combining medication expertise with family medicine under one roof, the team ensures continuity of treatment, better communication between providers and improved health outcomes for patients.
The clinic features five to six full-time family physicians supported by nursing staff, offering both traditional appointments and extended-hours walk-in services. This new integrated structure — anchored by Horizon Pharmacy — ensures comprehensive, accessible primary care for the growing Mill Woods population.
Addressing healthcare gaps
Janak and his partner's motivation for joining the Lakewood Covenant Community Health Centre stems from their daily observations of healthcare system challenges. As a founding member and director of the Alberta Independent Pharmacy Association, Janak understands the critical role that integrated care can play in improving patient outcomes.
"In my daily practice as a pharmacist, I have noticed a lot of gaps in patient care," says Janak. "I see the Covenant Wellness Community model as a solution for a lot of these problems that we come across as health professionals on a daily basis."
Combining multiple healthcare access points under one roof represents an innovative approach to addressing these gaps, from medication management to primary care co-ordination, says Janak.

Strategic community investment
Janak's decision to join the Lakewood Covenant Community Health Centre reflects both professional opportunity and community commitment. Having been involved in the project from very early on through Pharmacy Brands Canada and Colliers, he competed against major pharmacy chains to secure this strategic location.
"I think there were a few bigger brands competing to get this space — Rexall, Shoppers, Medicine Shoppe," says Janak. "But I think my presence and connection within the community, because I own other pharmacies all in the southeast of Edmonton, they decided to award it to me, and I'm very grateful for this opportunity."
Janak previously owned Lakewood Pharmacy directly across the street from the new Community Health Centre, which he sold to focus on this larger opportunity.
Collaborative healthcare innovation
What excites Janak most about the Lakewood Covenant Community Health Centre is its potential to transform how health care is delivered in one of Edmonton's fastest-growing neighbourhoods.
"It's multiple businesses, multiple healthcare access points under one roof. That is the new innovative idea that excites me more than anything," he says. "I think this is a great concept."
Janak’s enthusiasm reflects a broader understanding of how integrated care can improve patient outcomes while addressing the capacity challenges facing traditional healthcare delivery models.
As a proud banner partner of Pharmacy Brands Canada, Horizon Pharmacy & Medical Clinic represents the convergence of established community relationships, pharmaceutical expertise and innovative healthcare delivery. With Janak’s deep community connections and commitment to bridging healthcare gaps, the clinic is positioned to serve as a cornerstone of integrated care within the Lakewood Covenant Community Health Centre.
As southeast Edmonton continues to grow and evolve, Horizon's combination of experienced family physicians, comprehensive pharmacy services and collaborative care approach offers a glimpse into the future of community-based health care, where patients can access co-ordinated, convenient care that addresses their complete health and wellness needs in one accessible location.
